Need a Ride? Use this form to request a ride through our Elevate Transportation Program. We provide round-trip rides for medical appointments, errands, and other essential trips.

Please request your ride as early as possible so we have time to find a volunteer driver. Once you submit the form, we’ll get back to you with availability and a quote for the ride. Rides are based on availability and include a small fee.
